<p class="wp-block-paragraph">But not all peels are created equally.</p>

<p class="wp-block-paragraph">The most significant &#8220;brutal truth&#8221; is that many products marketed as at-home &#8220;peels&#8221; are, by clinical standards, merely cosmetic exfoliants.&nbsp;</p>



<p class="wp-block-paragraph">According to the <a href="https://mednexus.org/doi/pdf/10.1097/JD9.0000000000000363?download=true"><em>Expert Consensus on Chemical Peels</em></a>, concentrations of Alpha Hydroxy Acids (AHAs) ≤6% and Salicylic Acid (SA) ≤2% do not technically qualify as peels. They are designed for surface-level maintenance, not deep structural change.</p>



<ul class="wp-block-list">
<li><strong>AHAs (Glycolic/Lactic Acid):</strong> Consumer safety regulations generally cap at-home AHA concentrations at 10%. In a clinical setting, we routinely use concentrations reaching 70% to target the deeper dermis.</li>



<li><strong>BHAs (Salicylic Acid):</strong> Over-The-Counter products are capped at a 2% limit. While excellent for daily maintenance, they lack the caustic power of professional formulas.</li>



<li><strong>The pH and </strong><a href="https://drblink.co.za/collagen-stimulating-injectables-guide-skincare-after-30/"><strong>Collagen Factor</strong></a><strong>:</strong> Potential is dictated by pH. Most at-home products are &#8220;buffered&#8221; to a pH of approximately 4.0. While <a href="https://www.medicalnewstoday.com/articles/chemical-burn-on-face-from-skin-care">some studies</a> indicate that a pH of 4.0 can stimulate collagen production without the inflammation of a true peel, it cannot achieve the deep epidermal renewal of a clinical formula with a much lower, unbuffered pH.</li>



<li><strong>The Bactericidal Edge:</strong> For South Africans dealing with congestion from humid coastal air or city pollution, Glycolic Acid (GA) is the maintenance king. Clinically, GA possesses bacteriostatic and bactericidal effects on <em>P. acnes</em>, making it a powerful tool for preventing breakouts before they begin.</li>

<li><strong>Grade 2:</strong> A cloudy-white appearance, indicating the acid has affected the entire epidermis.</li>



<li><strong>Grade 3:</strong> Pure white frosting, signifying that the peel has reached the dermal layer.</li>

<h3 class="wp-block-heading">The Danger of DIY Neutralisation</h3>



<p class="wp-block-paragraph">A critical distinction lies in the chemistry of how these acids stop working.&nbsp;</p>



<p class="wp-block-paragraph">AHAs (like Glycolic acid) do <em>not</em> self-neutralise; they will continue to &#8220;digest&#8221; your skin tissue until an alkaline solution (usually 10.0%–15.0% sodium bicarbonate) is applied. In the clinic, we monitor this until the &#8220;foaming&#8221; stops.&nbsp;</p>



<p class="wp-block-paragraph">At home, users often rinse inadequately, leaving active acid to cause delayed-onset burns. Conversely, TCA is a self-neutralising agent that stops acting once it has coagulated with enough protein,but managing that depth requires the discerning eye of a specialist.</p>

<li><strong>The &#8220;Don’ts&#8221;:</strong> Avoid oils, butter, or unknown balms. Use only plain petroleum jelly to keep the wound moist and prevent a crust from forming too early.</li>

</ol>

Many of the most effective skincare actives are scientifically studied compounds derived from natural sources such as fruit sugars, plants, or even substances already present within the skin itself. Without the correct guidance, overusing active ingredients or combining them incorrectly can compromise the skin barrier, worsen inflammation, and aggravate conditions like pigmentation, rosacea and acne.</p>

<h3 class="wp-block-heading">Alpha-Hydroxy Acids (AHAs)</h3>



<p class="wp-block-paragraph">Alpha-hydroxy acids (AHAs) are water-soluble molecules that modulate the cohesion of the stratum corneum (the very top layer of your skin, where old cells that have been there for a while are). AHAs penetrate right into that top layer of your skin, making room for new skin cells.</p>



<p class="wp-block-paragraph">Glycolic and Lactic acids are the gold standards of this category due to extensive clinical investigation.</p>



<p class="wp-block-paragraph"><strong>Primary Evidence-Based Benefits:</strong></p>



<ul class="wp-block-list">
<li><strong>Improved Skin Moisturisation:</strong> AHAs increase hyaluronan biosynthesis in the dermis, enhancing water retention.</li>

<h3 class="wp-block-heading">Retinoids and Tretinoin</h3>



<p class="wp-block-paragraph">Retinoids, derivatives of Vitamin A, are the most potent tools for reversing photodamage. While over-the-counter Retinol is popular, prescription-strength Tretinoin (Retin-A) is the bioactive form that binds directly to nuclear receptors.</p>



<p class="wp-block-paragraph">Timeline of Expectations:</p>



<ul class="wp-block-list">
<li><strong>Weeks 1 to 4: The &#8220;Purge&#8221;</strong>: Accelerated turnover pushes pre-existing blockages to the surface causing possible breakouts.</li>



<li><strong>Weeks 4 to 8: Retinisation</strong>: A period of temporary dryness, peeling, and redness as the skin adapts.</li>



<li><strong>Weeks 12 to 24+:</strong> Meaningful improvement in fine lines and dermal collagen density.</li>
</ul>

<li><strong>Building Collagen</strong>: It is a mandatory &#8220;cofactor&#8221; or helper for the enzymes in your skin that build collagen. Without enough Vitamin C, these collagen-building machines cannot work properly to reduce the appearance of lines and wrinkles. </li>



<li><strong>Brightening and Evening Skin Tone</strong>: Vitamin C prevents dark spots by inhibiting an enzyme called tyrosinase. This enzyme is the &#8220;on switch&#8221; for producing melanin, the pigment that causes hyperpigmentation. </li>
</ul>



<p class="wp-block-paragraph">For Vitamin C to be effective, it must be in its most active form, L-ascorbic acid, or a stable derivative that the skin can convert into that form. L-Ascorbic Acid (L-AA) is the most potent but notoriously unstable form of Vitamin C, requiring a low pH (≤ 3.5) for penetration.</p>

<p class="wp-block-paragraph">We South Africans have a deep love for the sun — but over time, it doesn’t always return the favour. I recently treated a patient, a long-time Melkbos beach regular, who felt her skin was beginning to reflect years of sun exposure in a way that no longer aligned with how she felt.</p>



<p class="wp-block-paragraph">The approach wasn’t to overcorrect or intervene aggressively. Instead, it was about restoration — gently reawakening the skin’s natural vitality and refining its texture in a way that feels effortless and authentic. That philosophy sits at the heart of everything I do: achieving elegant, natural-looking results without surgery.</p>



<p class="wp-block-paragraph">One of the treatments I return to time and again for this is <a href="https://drblink.co.za/micro-needling-skinpen/">microneedling with the SkinPen</a>. It’s precise, sophisticated, and delivers beautifully consistent results.</p>
