What’s in HArmonyCa?
- Hyaluronic Acid (HA) is the same hydrating molecule used in traditional fillers. This gives you an instant volumising and lifting effect the moment it’s injected.
- Calcium Hydroxyapatite (CaHA) microspheres are tiny particles that act as a scaffold, encouraging your skin to build new collagen around them over the following months.
These two aren’t just mixed. They’re molecularly combined during manufacturing, so they work as one cohesive product rather than two separate ingredients fighting for space.
What Does HArmonyCa Do?
- Right away: You’ll see an immediate lift and improved definition, particularly along the cheekbones to jawline (what I call the “C-shape” zone).
- Over time: The CaHA stimulates your fibroblasts to produce Type III collagen, which gradually remodels into the stronger, denser Type I collagen. It also helps slow down the enzymes that break collagen down in the first place.
I deliver it with a fine cannula into deeper layers of the skin, typically in a single session that takes around 30 to 45 minutes.
Sculptra: The Slow Build That Lasts
Sculptra takes a completely different approach. There’s no instant volume here. This is a long game.
What’s in Sculptra?
It’s made from Poly-L-lactic acid (PLLA), a biocompatible material that triggers a controlled biological response in your tissue. Think of it as a gentle nudge to your body: “Hey, start making more collagen here.” Over time, your natural Type I collagen concentration can increase by up to 66.5%.
How Long Does Sculptra Take to Work?
- You won’t see real changes for about 4 to 6 weeks.
- Results typically peak 3 to 6 months after your final session.
- But those results can last 2 to 3 years, longer than almost anything else on the market.
One thing I tell my patients: Your face might look plumper right after treatment, but that’s just the sterile water used to reconstitute the product. It gets absorbed within a couple of days, and then you’re back to baseline while the collagen-building process kicks in behind the scenes.
Radiesse: Immediate Structure With Built-In Biostimulation
Radiesse occupies the middle ground between HArmonyCa’s hybrid approach and Sculptra’s pure biostimulation. It delivers an immediate structural lift and stimulates your collagen, but without the hydrating HA component you get with HArmonyCa.
What’s in Radiesse?
Radiesse is made of Calcium Hydroxyapatite (CaHA) microspheres, the same biostimulatory ingredient found in HArmonyCa, suspended in a carboxymethylcellulose (CMC) gel carrier. The formulation is roughly 30% CaHA and 70% gel by volume. CaHA is a naturally occurring mineral found in bones and teeth, so it’s highly biocompatible.
The key difference from HArmonyCa is what’s not in it: there’s no cross-linked hyaluronic acid. Instead of HA, Radiesse relies on its CMC gel for that initial lift. That gel provides immediate volume and structure.
What Does Radiesse Do?
- Right away: The CMC gel fills and lifts the treated area, giving you visible structural improvement from the moment it’s injected. Think sharper jawline, restored cheek volume, smoother nasolabial folds.
- Over time: As the gel is gradually absorbed, the CaHA microspheres go to work. They act as a scaffold, encouraging your fibroblasts to produce new collagen and elastin. The collagen that forms reinforces the initial lift, so results continue to improve even as the original gel carrier disappears.
Collagen stimulation typically peaks around the 12-week mark, which is why many patients tell me their results actually look better a few months after treatment than they did on day one.
Who Is Radiesse For?
Radiesse suits patients who want immediate definition and structure without committing to multiple sessions. It’s a strong choice if you’re dealing with:
- Moderate to deep nasolabial folds and marionette lines
- Volume loss in the cheeks, temples, or jawline where you want lift with definition
- Ageing hands, as Radiesse is FDA-approved for restoring volume to the back of the hands, which neither HArmonyCa nor Sculptra can claim
- Skin laxity on the neck, décolletage, or arms, when used in a hyperdilute formulation that prioritises biostimulation over volume
In its hyperdilute form, Radiesse shifts from a volumiser to a pure biostimulator. I dilute it with saline and lidocaine and inject it more superficially across larger surface areas. The CaHA microspheres still stimulate collagen and elastin production, but without adding bulk, so you get tighter, firmer skin rather than added volume. This makes it particularly useful for areas like the neck and décolletage where crepiness and laxity are the main concern, not volume loss.
How Long Does Radiesse Last?
Results typically last 12 to 18 months, with most patients booking a maintenance session around the 12-month mark. Because the collagen your body builds sticks around even after the product itself has been metabolised, the transition is gradual rather than a sudden drop-off.
Aftercare: HArmonyCa vs Sculptra vs Radiesse
Here’s a practical detail that catches a lot of my patients off guard.
Sculptra requires the 5-5-5 rule: massage the treated area for 5 minutes, 5 times a day, for 5 days. This prevents the PLLA particles from clustering together, which can cause small nodules under the skin. It’s not optional. It’s essential.
HArmonyCa is the opposite. After I do an initial clinical massage in the treatment room, you’re told to leave it alone. No touching, pressing, or massaging for 1 to 2 weeks. The hybrid gel needs to settle and integrate undisturbed.
Radiesse falls somewhere in between. There’s no multi-day massage protocol like Sculptra. Aftercare is straightforward: avoid pressing, rubbing, or massaging the area for the first 24 hours, skip strenuous exercise and excessive heat for 24 to 48 hours, and use a cold compress if you’re prone to swelling. Beyond that, it’s business as usual.
So if you’re someone who’d rather not add a daily massage routine to your schedule, both HArmonyCa and Radiesse have simpler post-treatment requirements than Sculptra.

You’re in it for the long haul and want deep, lasting restoration. You’re dealing with more significant volume loss: hollow temples, flat cheeks, overall deflation. You’re happy to commit to a few sessions and follow the aftercare protocol for results that could last up to three years. I recommend Sculptra.
You want immediate structure and definition without the HA component. You’ve got moderate to deep folds, you want a sharper jawline or more defined cheekbones, and you prefer a single-session treatment with straightforward aftercare. I recommend Radiesse.
You want to rejuvenate your hands. Volume loss on the back of the hands is one of the earliest giveaways of ageing, and Radiesse is the only one of these three with a specific approval for hand augmentation. Radiesse is the way to go.
You want to treat areas beyond the face. Skin laxity on the upper arms, décolletage, or the appearance of cellulite on the thighs and glutes. Sculptra is certified for off-face body indications. Radiesse, in its hyperdilute form, can also address skin laxity and crepiness on the neck, décolletage, arms, and hands. For body-focused concerns, Sculptra or hyperdilute Radiesse are your options, and the right choice depends on whether you need volume restoration or skin-quality improvement.
Safety and Reversibility: HArmonyCa vs Sculptra vs Radiesse
All three treatments are very safe when performed by a licensed medical practitioner. The usual suspects apply: some swelling, redness, and bruising at the injection site, which typically clear up within a week. The key difference is what happens if something needs correcting.
Is HArmonyCa Reversible?
Yes. HArmonyCa has a partial safety net: because the CaHA microspheres sit within a hyaluronic acid matrix, injecting hyaluronidase can dissolve that matrix. Once the carrier is cleared, the body’s own immune cells can break down the dispersed microspheres.
Is Sculptra Reversible?
No. PLLA can’t be dissolved with an enzyme, which is exactly why precise placement by a skilled injector is non-negotiable. It’s one of the reasons I’m so particular about technique with this product.
Is Radiesse Reversible?
No. Like Sculptra, the CaHA microspheres in Radiesse can’t be dissolved with hyaluronidase or any other enzyme. The CMC gel carrier is absorbed naturally by the body, and the CaHA integrates with your tissue as collagen forms around it.
This is why precision matters, and why choosing an experienced injector who understands facial anatomy is critical. That said, complications requiring reversal are rare with Radiesse when it’s placed correctly.
